Lighthouse – Forms Overview
There are three form types in Lighthouse: tasks, audits and issues. The different types of forms offer flexibility and are designed to cover a wide range of uses. Forms can be filled out in both web and mobile. Tasks and audits can be assigned to all locations, a specific location or area. We recommend that only your admins have access to add/edit your forms, as this limit helps you maintain standards and client requested compliance. When creating a form, you can copy an existing form and edit as appropriate, or you can add/build a new form. In new Lighthouse application, industry-based form templates default in your application to help you get started creating forms.
Tasks
Tasks are designed to capture simple repeated or scheduled activities where no follow-up action is required. They can be assigned to locations and areas so employees have the right form at their fingertips when they are at a location.
Example: If a task is assigned to a QR code or NFC tag, scanning automatically displays the form for the employee to complete.
Task examples
- Daily activity reports
- Sweep sheets
- Door/elevator/fire safety checks
- Post orders
Issues
Issues are designed to capture non-scheduled activity and/or activity where follow-up actions may be required. This type of form includes automatic reporting fields for status and assigned users. Notification preferences can be set based on issue status and the time-line of activity is recorded automatically. Also, issues include features where you can assign it to an individual to notify them of it, and you have the ability to change/track the status of the issue.

Audits
Audits are used for inspections, designed with a scoring/assessments feature. They allow you to set a custom scoring system (i.e. pass/fail. 1-5, etc.) and assign weighting or values to questions.

- Supervisor quality inspections
